Your Cleveland Dental Appointments

Initial Visit

We are anxious to meet with you and discuss your problem, desires, wishes and history. You may bring any xrays, records or models you have. This appointment is complementary, but does not include a clinical examination or xrays.

Your initial appointment will consist of a comprehensive dental evaluation. By thoroughly evaluating your entire oral condition, our doctors will be able to provide for you the most effective therapies for your personal dental situation.

We view each patient as an individual and will customize a specific treatment plan to provide you a healthy fully function and pain free mouth with a smile designed for you alone.

Please assist us by providing the following information at the time of your evaluation:

• Your referral slip and any radiographs if applicable
• A list of medications you are presently taking
• If you have dental insurance, bring your information. This will save time and allow us to help you process any claims.

IMPORTANT: All patients under 18 must be accompanied by a parent or guardian during this evaluation visit.

Please alert the office if you have a medical condition that may be of concern (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.), or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)

Scheduling

In an emergency, and if it is during office hours, call the office immediately and inform the staff of the problem so we can assist you in the most expedient way. After office hours, please call the regular office number and follow the instructions provided for emergency contact.
