Fortunately, most teeth that require endodontic treatment respond to conservative therapy. For those very few that do not respond, the patient will have continued discomfort which may be constant or sporadic.
For one of a myriad of reasons, the healing process that normally takes place in and around an endodontic tooth may not take place. When this occurs, only two options remain. Attempt to surgically treat the area by surrounding the end of the tooth's root and the end of the root itself, or by removing the tooth.
This treatment is quite difficult technically for the dentist, but it is actually quite simple for the patient. Using local anesthetic, the area to be treated is thoroughly numbed so there is no discomfort for the patient. The gum tissue in the infected area is lifted, and the root of the tooth is carefully inspected. If the root is not fractured, the area of infection surrounding the root is thoroughly cleaned. If required, the seal on the end of the root can then be enhanced. The tissues are closed with a stitch or two, and the process is complete.
Healing takes place over the next several weeks, and if successful, the patient’s pain will go away.
